This retrospective follow-up study examined longitudinal viral load trajectories among 376 HIV patients receiving first-line antiretroviral therapy (ART) in Central Ethiopia. The findings demonstrated a significant decline in viral load over time, confirming the effectiveness of ART. However, advanced WHO clinical stage, poor adherence, and lower baseline CD4 count were significantly associated with increased viral load, while CD4 ≥200 cells/mm³ predicted better virological suppression.
The study highlights the importance of early treatment initiation, adherence support, and continuous clinical monitoring to achieve sustained viral load suppression among people living with HIV.
